Abstract

Više od 20% populacije u industrijaliziranim zemljama pati od neke vrste preosjetljivosti na hranu. Imunološki posredovane reakcije na hranu nazivamo alergijom, a ne-imunološki posredovane intolerancijom na hranu. Alergije na hranu dijele se na IgE posredovane i ne-IgE posredovane, a intolerancije na enzimske, farmakološke, one uzrokovane poremećajem u transportu i nedefinirane. Ne postoji jedinstven test kojim se može utvrditi alergija ili intolerancija na hranu pa je potreban strukturirani multidisciplinarni dijagnostički pristup. Zlatni standard među dijagnostičkim postupcima je dvostruko slijepi placebom kontrolirani provokacijski test na hranu (eng. Double-blind placebo-controlled food challenge, DBPCFC). Jedini učinkoviti način liječenja je isključenje određene namirnice/a iz prehrane. Kod preosjetljivosti na hranu koja je nužna za rast i razvoj djeteta treba dati adekvatne zamjene.

Abstract (english)

More than 20% of the population in industrialized countries suffer from some type of food hypersensitivity. Immune-mediated reactions to food are called food allergies and non immune-mediated reactions, food intolerances. Food allergies are divided into IgE-mediated and non-IgE-mediated, and food intolerances into enzymatic, pharmacological, those caused by transport defect and undefined. There is no unique test that can determine food allergy or intolerance. A structured multidisciplinary diagnostic approach is required. The gold standard among the diagnostic procedures is the double-blind placebo-controlled food challenge (DBPCFC). The only effective treatment is the exclusion of the certain food/s from the diet. In hypersensitivity to food, which is essential for the growth and development of a child, an adequate substitute should be given.
