Praćenje nutritivnog statusa i pridržavanja bezglutenske dijete u odraslih bolesnika sa celijakijom

Abstract

Celijakija je bolest koju karakterizira trajna nepodnošljivost glutena kod genetski predisponiranih pojedinaca. Cilj ovoga rada bio je procijeniti nutritivni status i prehrambene navike, te iz toga provjeriti pridržavanje bezglutenske prehrane. Istraživanje je provedeno antropometrijskim, dijetetičkim i laboratorijskim ispitivanjima, a u njega su bili uključeni ambulantni pacijenti Kliničkog bolničkog centra Zagreb – Rebro. Od ukupno 65 pacijenata iz baze s dijagnosticiranom celijakijom u odrasloj dobi, 21 pacijent (5 muškaraca i 16 žena) činio je eksperimentalnu skupinu. Antropometrija je uključivala mjerenje tjelesne visine i mase, te izračun indeksa tjelesne mase, dijetetičke metode uključivale su 3- dnevni dnevnik prehrane, a laboratorijski i dijagnostički parametri određivanje protutijela na tkivnu transglutaminazu, patohistološki nalaz crijevnih resica te analize HLA-DQ2 i DQ8 heterodimera. Prema rezultatima, ispitanici su svrstani u kategoriju adekvatne tjelesne mase. Kod muških ispitanika viši je unos ukupnih masti i ugljikohidrata nego kod žena. Kod laboratorijskih parametara, primijećeno je ukoliko se pacijent pridržava bezglutenske prehrane, dolazi do sniženja vrijednosti tTg-a u odnosu na prvo mjerenje te do histološkog oporavka atrofije sluznice. Analizom dnevnika prehrane i rezultata histološkog oporavka atrofije sluznice uočeno je da se svi ispitanici eksperimentalne skupine pridržavaju bezglutenske prehrane, ali ne i principa pravilne prehrane.

Abstract (english)

Celiac disease is a disease characterized with a permanent gluten intolerance at genetically predisposed individuals. The aim of this study was to estimate nutritional status and dietary habits and test adherence to a gluten-free diet. The survey was conducted using anthropometric, dietary and laboratory methods and it included ambulatory patients from the Clinical Hospital Centre Zagreb- Rebro. Of total 65 patients from the base with diagnosed celiac disease in their adulthood, 21 of patients (5 men and 16 women) consisted an experimental group. Anthropometry included a body height, body weight and body mass index, dietetic methods included 3– daily food diary, and laboratory measurements detect antibodies against the gluten epitopes –tTg, pathohistological finding of intestinal villi, and HLA DQ2 and DQ8 heterodimer analysis. All subjects are in the category of normal (preferable) body weight. Among men subjects the import of total fat and carbs is higher than for women. With laboratory parameters it is noticed that in case the patient follows gluten-free diet, it comes to depreciation of tTg’s value from the first measuring and to histological recovery of mucosa atrophy. Analysis of the food diary and histologic mucosal atrophy recovery results it is observed that all respondents experimental groups follows the gluten-free diet, but not the principles of proper nutrition.
