Abstract

Crohnova bolest (CB) i ulcerozni kolitis (UK) dva su glavna oblika upalnih bolesti crijeva, a terapija biološkim lijekovima pokazala se najučinkovitijom za kontrolu simptoma i postizanje kliničke remisije. Cilj ovoga rada bio je odrediti sastav tijela te potrošnju energije u mirovanju bolesnika koji se liječe biološkom terapijom. Provedeno istraživanje obuhvatilo je 74 ambulantna pacijenta Kliničkog bolničkog centra Zagreb (52 ispitanika iz CB skupine te 22 ispitanika iz UK skupine). Metodom bioelektrične impedancije dobiveni su podaci o sastavu tijela ispitanika – tjelesna masa (TM), indeks tjelesne mase (ITM), udio masnog tkiva (%MT), visceralno masno tkivo (VMT) te mišićna masa (MM). Kod 32% ispitanika zabilježen je povećani ITM, dok 46% ispitanika ima povišen %MT. Indeksom skeletnog mišićja (SMI) utvrđena je prisutnost umjerene sarkopenije kod 32 ispitanika, od čega je bilo 30 muškaraca, χ2=38,11, p<0,00001. Metodom indirektne kalorimetrije (IC) dobivene su vrijednosti potrošnje energije u mirovanju (RMR), pri čemu je izmjerena RMR (1570 kcal) prosječno značajno niža od predviđene (1640 kcal), t(73)=-3,023, p<0,05. Rezultatima ovog istraživanja kod ispitanika je utvrđena značajna zastupljenost umjerene sarkopenije, povišena tjelesna i masna masa te blago snižena potrošnja energije u mirovanju, što se može povezati s utjecajem biološke terapije.

Abstract (english)

Crohn’s disease (CD) and ulcerative colitis (UC) are the two major forms of inflammatory bowel disease and biological therapy has proven to be the most effective in controlling the symptoms and inducing clinical remission. The aim of this study was to determine body composition and resting energy expenditure of patients treated with biological therapy. The study includes 74 patients at the Clinical Hospital Center Zagreb (52 patients from the CD group and 22 from the UC group). The bioelectrical impedance method obtained data on the body composition of the subjects – body mass (BM), body mass index (BMI), fat mass (FM), visceral fat rating (VFR) and muscle mass (MM). Increased BMI was observed in 32% of the patients, while 46% of them had an increased FM. The skeletal muscle index (SMI) identified the presence of moderate sarcopenia in 32 patients, of which 30 were men, χ2=38,11, p<0,00001. Indirect calorimetry (IC) obtained the value of resting energy expenditure (REE), with the measured REE (1570 kcal) on average significantly lower than predicted (1640 kcal), t(73)=-3,023, p<0,05. The results of this study revealed a significant prevalence of moderate sarcopenia, increased body mass and fat mass, and slightly lowered resting energy expenditure, which may be related to the impact of biological therapy.
