Abstract

Pacijenti s kolorektalnim karcinomom u usporedbi s drugim vrstama malignih tumora imaju jednu od najviših stopa malnutricije što negativno utječe na ishode liječenja. Cilj ovog rada bio je procijeniti nutritivni status pacijenata s kolorektalnim karcinomom i utvrditi postoji li povezanost s omjerom izvanstanične vode i ukupne tjelesne vode (ECW/TBW) u trenutku postavljanja dijagnoze. U istraživanje je ukupno bilo uključeno 104 pacijenta oboljelih od kolorektalnog karcinoma kojima je neposredno nakon postavljanja dijagnoze određen nutritivni status NRS 2002 upitnikom, određeni biokemijski parametri te izmjeren sastav tijela bioelektičnom impendancijom. U trenutku postavljanja dijagnoze 24 % ispitanika ima povećani rizik od malnutricije, čak 53 % ispitanika ima snižene vrijednosti željeza, a 35 % albumina. Prema rezultatima istraživanja pacijenti s nižim omjerom ECW/TBW bili su mlađi i imali su značajno veći fazni kut (PhA), sarkopenijski indeks (SMI) te serumski albumin i hemoglobin u odnosu na pacijente s višim omjerom ECW/TBW. Omjer ECW/TBW može biti koristan pokazatelj nutritivnog statusa u osoba s kolorektalnim karcinomom.

Abstract (english)

Compared to other types of malignant tumors, patients with colorectal cancer have one of the highest rates of malnutrition, which negatively affects treatment outcomes. This work aimed to evaluate the nutritional status of patients with colorectal cancer and determine whether there si an association with the extracellular to total body ratio (ECW/TBW) at the time of diagnosis. A total of 104 patients with colorectal cancer were included in the study, whose nutritional status was determined by the NRS 2002 questionnaire immediately after diagnosis, biochemical parameters were determined and body composition was measured by bioelectric impedance. At the time of diagnosis, 24 % of the subjects had an increased risk of malnutrition, as many as 53 % of subjects had decreased iron values, and 35 % of albumin. According to the research results, the group of patients with a lower ECW/TBW ratio was younger and had significantly higher phase angle (PhA), skeletal muscle index (SMI), serum albumin, and hemoglobin than the group with a higher ECW/TBW ratio. ECW/TBW ratio may be a useful indicator of nutritional status in people with colorectal cancer.
