Abstract

Kalprotektin, također poznat kao mijeloid-povezan protein MRP-8/MRP-14, S100A8/A9, kalgranulin A i B te L1, otpušta se iz aktiviranih granulocita i monocita/makrofaga u vrijeme upale. U slučaju akutne infekcije makrofagi koji predstavljaju stanice mijeloidne loze diferenciraju u proupalne makrofage (M1) koji su potrebni za učinkovito suprotstavljanje patogenima, izlučuju različite citokine, kemokine i druge upalne medijatore. Cilj ovog rada bio je provjeriti može li se kalprotektin u stolici koristiti kao prediktivni biomarker aktivnosti bolesti u juvenilnom idiopatskom artritisu gdje mijeloidne stanice imaju ključnu ulogu. Juvenilni idiopatski artritis je dugotrajna upala zglobova nepoznate etiologije koja počinje prije navršene 16. godine. Nasljedni faktori mogu povećati rizik od nastanka bolesti. Tipični simptomi upale u juvenilnom idiopatskom artritisu su: bol, oteklina i slabija pokretljivost zgloba, koji traju najmanje šest tjedana. U liječenju bolesti koriste se različiti lijekovi poput nesteroidnih protu-upalnih lijekova, glukokortikoida, metotreksata kao i anticitokinski lijekovi (tzv. biološka terapija). U ovom istraživanju pratili smo 32 bolesnika s dijagnozom juvenilnog idiopatskog artritisa kojima je izmjerena vrijednost kalprotektina u stolici te nismo potvrdili korelaciju između aktivnosti bolesti i koncentracije kalprotektina.

Abstract (english)

Calprotectin, also known as myeloid related protein MRP-8/MRP-14, S100A8/A9, calgranulin A i B and L1, is released from activated granulocytes and monocytes/macrophages during inflammation. In a case of acute infection macrophages differentiate in proinflammatory macrophages (M1) necessary to efficiently confront invading pathogen, secreting different cytokines, chemokines and other inflammatory mediators. The aim of this study was to analyze if calprotectin in stool serve as predictive marker of disease activity in juvenile idiopathic arthritis where myeloid cell have a crucial role. Juvenile idiopathic arthritis is long-term joint inflammation of unknown etiology that begins before of age sixteen. Hereditary factors can increase the risk of disease. Typical symptoms of inflammation in juvenile idiopathic arthritis are pain, swelling and lower mobility of the joint, which last for at least sixth weeks. Various drugs such as nonsteroidal anti-inflammatory drugs, glucocorticoids, methotrexate and the anticytokine drugs (so-called biological therapy) are used for treatment . In this study, we measured the concentration of calprotectin in stool in 32 patients diagnosed with juvenile idiopathic arthritis and we did not prove correlation between disease activity and calprotectin concentrations.
