Abstract

U opeklinskih je bolesnika povećan stupanj smrtnosti dijelom i posljedica nepravovremeno uvedene prehrane unatoč svim ostalim korištenim načinima liječenja. Enteralnu prehranu kod opeklinskih bolesnika treba započeti unutar dvadeset i četiri sata od nastanka opekline. Čak i neznatne količine hrane u crijevnome lumenu , manje od 200 ml unijete unutar prvih dvadeset i četiri sata od nastanka opekline mogu spriječiti propadanje crijevnih resica a time i razvoj sistemskoga upalnoga odgovora s bakterijskom translokacijom koja se i dalje smatra najvažnijim izvorom septičkih komplikacija. Prednosti tako rano uvedene enteralne prehrane su: održavanje cjelovitosti i funkcije crijevne sluznice, sprečavanje oksidativnoga stresa, smanjivanje hipermetabolizma kao i komplikacija opeklinske bolesti kao što su sepsa i višeorgansko zatajenje. Ranoj enteralnoj prehrani kod opeklinske bolesti treba pristupiti kao osnovnom načinu liječenja, a ne kao potpornoj terapiji. Tada su i njezini rezultati s obzirom na broj komplikacija, stupanj smrtnosti, trajanje bolničkog liječenja i ukupne troškove liječenja obećavajući što je i dokazano u ovom istraživanju.

Abstract (english)

The increased mortality rate in burn patients is partly the consequence of ill-timed implementation of nutrition despite all other therapeutic methods. Enteral nutrition should be initiated within 24 hours after burn injury. Even a small quantity of food in the intestines less than 200 ml delivered within the first 24 hours after burn injury may prevent atrophy of intestinal villi and thus inhibit the development of systemic inflammatory response associated with bacterial translocation, which is still considered the main source of septic complications. The advantages of early enteral feeding are: the maintenance of bowel mucosa integrity, prevention of oxidative stress, reduction of hypermetabolism and burn injury related complications (sepsis, MOF). Early enteral nutrition should be applied in burn patients as the basic method of treatment and not a supportive therapeutic measure. If applied in this way, the results are promising with regard to the number of complications, mortality rates, length of hospital stay and total costs of treatment. This was confirmed by our study.
