Utvrđivanje glutena ELISA metodom u bezglutenskim obrocima prikupljenim duplikat dijetom u djece na striktnoj bezglutenskoj dijeti

Abstract

Striktna bezglutenska dijeta osnovni je tretman za liječenje celijakije. Unos glutena uzrokuje oštećenja crijevnih resica kod takvih osoba, do kojeg može doći i zbog nenamjernog unosa glutena zbog česte kontaminacije namirnica glutenom. Cilj ovog rada je bio utvrditi nalazi li se gluten u cjelodnevnim obrocima djece s celijakijom koja provode bezglutensku dijetu te kakva je kakvoća njihove prehrane s obzirom na nacionalne preporuke. Uz pomoć roditelja metodom duplikat dijete prikupljeni su cjelodnevni bezglutenski obroci (n=16) za djecu (dobi 2-18 godina) tijekom 2 odabrana dana uz vođenje dnevnika prehrane. ELISA metodom određeno je kako nijedan analizirani cjelodnevni obrok nije sadržavao glutena više od limita kvantifikacije metode (< 5ppm). Analizom dnevnika prehrane utvrđeno je da je bezglutenska prehrana djece s celijakijom neuravnotežena. Nedovoljan unos prehrambenih vlakana utvrđen je kod 75% ispitanika, previsoki unos natrija kod 63% ispitanika, a dnevni energijski udio masti veći od preporuka u 50% ispitanika. Analizirani bezglutenski obroci sigurni su za konzumaciju djece oboljele od celijakije obzirom da ne sadrže gluten, no potrebno je dodatno educirati roditelje kako bi prehrana djece bila i nutritivno zadovoljavajuća.

Abstract (english)

A strict gluten-free diet is a basic treatment for celiac disease. Gluten intake causes damage to the intestinal villi in such people. This kind of damage can occur because of the unintentional intake of gluten due to the increasing contamination of foods with gluten. The aim of this study was to determine whether gluten is found in whole-day meals of children with celiac disease on a gluten-free diet and to determine the quality of their diet compared with national dietary recommendations. With the help of parents of the participants using the duplicate diet method, children (2-18 years) whole-day gluten-free meals (n=16) for 2 days combined with the food diary were collected. Using the ELISA method it was determined that none of the analyzed whole-day meals contained gluten more than the prescribed permissible levels (<5 ppm). The analyzed food diaries showed that the gluten-free diet of the children was unbalanced. Insufficient dietary fibre intake was found in 75% of subjects, high intake of sodium in 63% of subjects, and the daily energy content of fat higher than recommended in 50% of subjects. The analyzed gluten-free meals are safe for consumption by children with celiac disease due to the minimal detected amount of gluten in them but it is necessary to further educate parents so that the diet of children becomes richer in nutrients.
