Abstract

Prema današnjoj klasifikaciji, dijabetes tipa 1 definira se nedostatkom inzulina sa ili bez naznake autoimune destrukcije beta-stanica Langerhansovih otočića. U mlađih ljudi često počinje naglo, s ispoljenim simptomima hipoglikemije. U starijih je početak bolesti obično sporiji, izgleda kao da se radi o dijabetesu tipa 2. Striktnom kontrolom glikemije može se odložiti nastanak dijabetičkih komplikacija. Ona se provodi bazal-bolus liječenjem: višestrukim injekcijama inzulina ili kontinuiranom potkožnom infuzijom uz samokontrolu glikemije i prilagođavanje doze rezultatima liječenja. Cilj ovog rada je bio odrediti utjecaj samokontrole na glikirani hemoglobin u pacijenta s dijabetesom tipa 1. Glikirani hemoglobin je indeks za kontrolu glukoze u krvi kroz dulje vremensko razdoblje, a koristi se za procjenu efikasnosti kontrole dijabetesa. Pokazalo se da je samokontrolom moguće pozitivno utjecati na glikirani hemoglobin i njegovo smanjenje te efikasno kontrolirati dijabetes tipa 1.

Abstract (english)

According to the current classification, type 1 diabetes mellitus is defined as a condition caused by a lack of insuline with or without the signs of autoimmune destruction of beta cells in the islets of Langerhans. In younger patients, the disease onset is usually acute, with pronounced symptoms of hyperglycemia. In the elderly, the disease onset is usually slower, resembling type 2 diabetes. Strict glycemic control may postpone diabetic complications. The glycemic control is maintained by basal-bolus insulin therapy with multiple daily injections or continuous subcutaneous insulin infusion, which requires self-monitoring of blood glucose and insulin dose adjustments based on the glucose level. The purpose of this paper was to determine the effect of self-control on glycated hemoglobin in patients with type 1 diabetes. Glycated hemoglobin is measured to identify the average plasma glucose concentration over prolonged periods of time and it is used to asses efficiency of diabetes control. It was shown that self-control can positively affect glycated hemoglobin and its reduction and help to controle type 1 diabetes.
