Abstract

Određivanje antibakterijske aktivnosti antibiotika korištenih u suzbijanju uobičajenih patogena urinarnog trakta se provodi najčešće in vitro metodom disk difuzije. Međutim in vitro testovi ne odražavaju situaciju in vivo. U ovom radu uzorci urina su testirani na baktericidnu aktivnost na niz patogena urinarnog trakta. Rezultati ex vivo testova (titrovi) su uspoređeni s in vitro testiranjem. Svi antibiotici su pokazali značajnu baktericidnu aktivnost u urinu na ne ESBL Escherichia coli i Proteus mirabilis. Stariji cefalosporini su pokazali baktericidnu aktivnost u urinu na Escherichia coli, Klebsiellla pneumoniae i Proteus mirabilis, ali njihovi titrovi su ubrzano opadali tijekom intervala doziranja. Fluorokinoloni su pokazali visok i značajan nivo baktericidne aktivnosti u urinu prema svim gram-negativnim bakterijama i Staphylococcus saprophyticus. Linezolid je jedini antibiotik koji je pokazao značajnu baktericidnu aktivnost u urinu prema Enterococcus faecium. Prosječna baktericidna aktivnost se može predvidjeti iz in vitro testiranja, ali može se očekivati da će biti pacijenata s nižom razinom baktericidne aktivnosti što je posljedica promjenjive koncetracije antibiotika u urinu.

Abstract (english)

The antibacterial activity of antibiotics against common urinary tract pathogens is determined by in vitro testing, usually by disc diffusion test. However, in vitro antibiotic susceptibility tests cannot reflect the situation in vivo. In this investigation , the urine samples were examined for their bactericidal activity against a range of relevant urinary tract pathogens. The results of ex vivo (titers) were compared with in vitro testing.All antibiotics showed a significant urinary bactericidal activity against non-ESBL Escherichia coli and Proteus mirabilis. Older cephalosporins had marked urinary bactericidal activity against non-ESBL Escherichia coli, Klebsiellla pneumoniae and Proteus mirabilis, but their titers decreased rapidly during the dosing interval. Fluoroquinolones displayed high and persisting levels of urinary bactericidal activity against all gram-negative bacteria and Staphylococcus saprophyticus. Linezolid was the only antibiotic showing significant level of urinary bactericidal activity against Enterococcus faecium.Average urinary bactericidal activity can be predicted from in vitro susceptibility testing, but we except that there will be patients with low level of urinary bactericidal activity due to variable antibiotic concetrations in urine.
