Abstract

Percentilne krivulje su jedna od najčešće korištenih metoda procjene uhranjenosti kod djece. Razlog praćenja uhranjenosti djece je procjena nutritivnog statusa i provjera je li rast i razvoj unutar granica adekvatnih s obzirom na određenu dob. Najčešće se koriste CDC (Center for Disease Control and Prevention) i WHO (World Health Organization) percentilne krivulje, a Hrvatska ima i nacionalne krivulje. Cilj ovog istraživanja je usporediti hrvatske (HR) krivulje s CDC i WHO krivuljama, koristeći Bland-Altman metodu, kako bi se mogle uvidjeti sličnosti/razlike HR, CDC i WHO krivulje koje se koriste u sustavu javnog zdravstva. Krivulje su uspoređivane na temelju antropometrijskih veličina ispitanika (djeca od 6-9 godina, iz Zagreba i Dubrovnika). Rezultati su prikazani izračunatim prosjecima antropometrijskih veličina i pripadnih percentilnih vrijednosti. Prikazane su frekvencije distribucije kao i normalnost razdiobe promatranih skupova podataka. Sličnosti i razlike utvrđene su primjenom t-testa i Bland-Altmanove metode. Bland-Altman metoda je pokazala sličnost između svih krivulja, dok se na prikazima frekvencija vide razlike između HR, CDC te WHO krivulja. Rezultati pokazuju kako bi se udio pretile djece korištenjem HR krivulja potencijalno podcijenio, ali ne statistički značajano te Bland-Altman metoda za HR krivulje ne pokazuje značajna odstupanja.

Abstract (english)

Growth charts are commonly used method for the assessment of the nutritional status of children. The reason for monitoring the nutritional status of children is to assess nutritional status and to verify adequacy of the growth and development according to given age. CDC (Center for Disease Control and Prevention) and WHO (World Health Organization) growth charts are used the most often, and Croatia also has its national growth charts. The aim of this study was to compare the Croatian (HR) growth charts with the CDC and WHO charts, using the Bland-Altman method, in order to assess similarities/differences of HR, CDC and WHO charts that are used in the public health system. Growth charts comparison is based on anthropometric variables of subjects (6 to 9 years old children, from Zagreb and Dubrovnik). The results are presented with calculated averages of anthropometric variables and corresponding percentile values. Presented are also the frequency distributions as well as the normality of the distribution for the observed data sets. To detect similarities and differences, the T-test and the Bland-Altman method was applied. The Bland-Altman method showed similarity between the growth curves, while the displayed frequencies showed differences between Croatian and CDC or WHO charts. Results show that the share of overweighed children could be underestimated if the HR growth curves are used, but the differences are not statistically significant and Bland-Altman method shows no significant discrepancies for HR curves.
