Abstract

Crohnova bolest je idiopatska kronična upalna bolest crijeva karakterizirana transmuralnom upalom sluznice gastrointestinalnog trakta. Iako može zahvatiti bilo koji dio gastrointestinalnog trakta od usta do anusa, najčešće zahvaća terminalni ileum i kolon. Prevalencija i incidencija su u porastu diljem svijeta. Tipični simptomi bolesti su proljev, gubitak težine, bol u abdomenu, gubitak teka, malnutricija. Malnutricija je ujedno i najveći izazov ove bolesti s obzirom da pogađa 23 % izvanbolničkih i gotovo 85 % hospitaliziranih pacijenata. Glavni cilj liječenja ove doživotne bolesti je indukcija i održavanje remisije. Dijetoterapija i nutritivna potpora važne su, ne samo za prevenciju i liječenje malnutricije, već i kao potporna terapija bolesti, s naglaskom na individualni pristup. Velik broj pacijenata suočen je i s nedostatkom vitamina topivih u mastima i vodi te neadekvatnim mineralnim statusom. Cilj ovog završnog rada je dati pregled dodataka prehrani čiji se deficiti pojavljuju kod oboljelih od Crohnove bolesti te ispitati mogući utjecaj na prevenciju i liječenje bolesti.

Abstract (english)

Crohn's disease is an idiopathic, chronic inflammatory bowel disease characterized by transmural inflammation of the mucosa of the gastrointestinal tract. Although it can affect any part of the gastrointestinal tract from the mouth to the anus, most commonly it affects terminal ileum and colon. Prevalence and incidence are increasing worldwide. Typical disease symptoms are diarrhea, weight loss,abdominal pain, loss of appetite and malnutrition. Malnutrition is also the biggest challenge of this disease, considering that it affects 23 % of outpatients and almost 85 % of hospitalized patients. The main goal of treatment for this lifelong disease is the induction and maintenance of remission. Diet therapy and nutritional support are important, not only for the prevention and treatment of malnutrition, but also as supportive therapy of the disease, with an emphasis on an individual approach. A large number of patients are also faced with a lack of fat- and water-soluble vitamins and inadequate mineral status. The aim of this undergraduate thesis is to provide an overview of dietary supplements whose deficits appear in patients with Crohn's disease and to examine the possible impact on the prevention and treatment of the disease.
