Abstract

Crohnova bolest je kronična upalna bolest crijeva čija se pojava objašnjava promjenom u imunološkom odgovoru organizma na crijevnu mikrobiotu uslijed prisutnosti okolišnih faktora uz genetsku predispoziciju. Prehrana kao jedan od okolišnih faktora, a posebno povećani unos ultra-procesirane hrane, može utjecati na mikrobiotu, a time i na samu bolest. Cilj istraživanja je prikazati kliničke, antropometrijske i prehrambene karakteristike adolescenata oboljelih od Crohnove bolesti te utvrditi postoji li povezanost između promatranih parametra i konzumacije hrane različitog stupnja procesiranja. U istraživanju je sudjelovalo 20 ispitanika s dijagnozom Crohnove bolesti koji su se liječili u Klinici za dječje bolesti Zagreb. Korištene su antropometrijske, biokemijske i dijetetičke metode te metoda indirektne kalorimetrije. Hrana je kategorizirana prema SIGA klasifikaciji u 8 kategorija s obzirom na stupanj procesiranja. Cjelodnevnom unosu energije kod adolescenata najviše doprinosi ultra-procesirana hrana čiji udio iznosi 48,5 % (37,1 – 75,0 %) te su utvrđeni deficiti vitamina A, D, E, folne kiseline i cinka. Povezanost je utvrđena između lokacije upale i unosa ultra-procesirane hrane.

Abstract (english)

Crohn's disease is an inflammatory bowel disease. It’s occurrence is explained by a change in the immune response to the intestinal microbiota due to the presence of environmental factors in addition to genetic predisposition. One of the important environmental factors is diet. Increased intake of ultra-processed food, due to processing and added ingredients, can affect the microbiota and the disease. The aim of the research is to show clinical, anthropometric and nutritional characteristics of adolescents with Crohn's disease and to determine whether there is a connection between certain parameters and their diet patterns. 20 subjects with Crohn's disease who were treated at the Children's Hospital Zagreb took part in the research. Anthropometric, biochemical and dietetic methods and indirect calorimetry were used. Food is categorized with SIGA classification into 8 categories according to the degree of processing. Ultra-processed food contributes the most to the energy intake of adolescents. Deficiencies of vitamins A, D, E, folic acid and zinc have been detected. The location of the inflammation is correlated with the intake of ultra-processed food.
